^13th Annual Easter Egg Hunt

Join us for the 13th Annual Easter Egg Hunt on Holy Saturday. Search for eggs, make some Easter crafts, take your picture with the Easter Bunny, and enjoy refreshments.
All Childen in the parish, students and their parents are invited to this FREE event!
Children must be accompanied by adults at all times. Donations are accepted.

  I (we) the parent(s) and/or guardians of my (our) minor child(ren) listed above on this form, do hereby consent and authorize the release, publication, dissemination, distribution, use, and/or reproduction of any and all photographs taken of my (our) son/daughter Euclid Life Teen Easter Egg Hunt by a volunteer, chaperone or representative of Ss. Robert and William/St. John of the Cross Parish. The photographs may be used for a display on the parish kiosk, bulletin boards, in slide shows, parish website and possibly on a future youth ministry brochure. This Release and Authorization acknowledges that all photographs, negatives, positives, and prints shall constitute the property of Euclid Life Teen and may be used by Euclid Life Teen without any compensation or further notice to me (us) or to my son/daughter.
  I hereby request permission for the above named child/children to attend this event and I consent to the child’s participation in this event. I hereby assume all risks in connection with this event and I further release discharge, and/or otherwise indemnity the Diocese of Cleveland, the Bishop of the Roman Catholic Diocese of Cleveland, Ss. Robert and William/St .John of the Cross Parish employees and volunteers from all claims, judgments, liability by or on behalf of my child, myself and my spouse for any injury or damage due to the child’s participation in this event including all risks connected therewith whether foreseen or unforeseen. Furthermore, I acknowledge that it is my responsibility to provide adequate health insurance for my child/children.
